Leak Adjustment Application

Have a Leak? Leak adjustment forms can be filled out for leaks that are a hidden underground such as a service pipe leak, or hidden leak in crawl space and concrete floors that does not flow to the drain. Any claim for an adjustment will only be made within 90 days after the date of the bill in which the leak first appeared. Fill this form out and submit with copies of receipts of plumbing supplies purchased, plumbers invoice, or approved photos of the repair along with the application. Please note: Adjustments will not be made based on usage resulting from toilet leaks, vandalism, mechanical failure, neglect, filling a pool, washing vehicles, and watering sod. Also, if the meter at the property has been accessed, tampered with, or turned on or off by anyone unauthorized which resulted in loss of water will not be qualified for a leak adjustment.

Fill and submit this form and email receipts, photos, and invoices to customerservice@ewsu.com or come in person to drop off back up documentation for the leak adjustment form. If submitting back up documentation via email then please include your tracking number received upon completion and submission of form. If Dropping off the forms in person, then please include address along with tracking number of application.
